What to Provide

The background check report for a sample of employees hired during the audit period.

NOTE: The audit team will communicate sample selections once they have been finalized. Please refrain from attaching evidence to this ER until samples have been communicated.

Evidence Format

Word/PDF document, exported images in a common image file type (i.e. .jpg, .png, .pdf), Excel/.csv export from the source system

Additional Guidance

If a background check cannot be provided for any sampled employees, please explain why in the Evidence Description.

The evidence should demonstrate the:

  • Name of employee

  • Date background check was initiated

  • Date background check was completed

You do not need to provide the entire background check report, just enough of the document to demonstrate completion (e.g., a redacted cover page).
